Office Policies

Cancellation Policies

If you are a NEW patient and are booking your appointment for the first time with Dr. Lesley Rabach or Dr. Morgan Rabach, a valid Credit Card number will be taken either via CC Authorization Form or over the phone in order to hold your appointment time. If you cancel your appointment within 24-hours, on the same day, change your appointment, or do not show up to your appointment, your card will be charged a $500 fee. MONDAY appointments must be canceled by phone on the previous FRIDAY.

If you are an ESTABLISHED patient with Dr. Lesley Rabach or Dr. Morgan Rabach and you either cancel your appointment within 24 hours, move your appointment, or do not show up to your appointment, you will be charged a $500 fee upon your next visit.

New Patient Appointment Bookings – Same-Day Appointments

If you are a new patient booking an appointment with Dr. Lesley Rabach / Dr. Morgan Rabach for a same-day appointment, a credit card number will be taken to hold your time. If you do not show up to your appointment, a $500 no-show fee will be charged to your credit card.

Tardy Policy

If you are booked for treatment and you are more than 10 minutes late, there is NO GUARANTEE that you will receive same-day treatment. Please arrive on time or 5 minutes early to your appointment.

Deposits for Surgical Procedures and Ultherapy

Any appointment that requires one or more hours on the books will require a $1000, NON-REFUNDABLE deposit in order to be scheduled.

Cancellation Policy for Patch Test, Microneedling, or Sculptra

If you are a patient with Dr. Lesley Rabach or Dr. Morgan Rabach and you either cancel your appointment within 24 hours, move your appointment, or do not show up to your appointment, you will be charged a $200 fee for Patch Testing, Microneedling appointments or Sculptra appointments.

Payment Policy

Payment is due in full at the time of treatment. If paying by credit card, the card must be in the patient’s name. If not, then a credit card authorization form must be completed, signed, and returned to LM Medical NYC staff in order to receive treatment.

Consultation Fee with Dr. Lesley Rabach / Dr. Morgan Rabach

Any new patient seeking a first time consultation with Dr. Lesley Rabach / Dr. Morgan Rabach will require a $350 fee. This will go towards any of the patient’s first treatment cost or applied to a surgical procedure if booked on the day of the consultation.

Photo Policy

Please be prepared to remove your makeup upon arrival for pre-treatment photos. All patients are required to have photos taken in the office at every visit. Make up can typically be reapplied immediately after treatment.
